Notification of moving in (when you move into Oshu from outside the city)

When to notify	Within 14 days of moving to Oshu (Procedures aren't possible before moving in.）
Who can notify	The person who moved or someone in their household (For a non-household member notifying on someone else's behalf, a notification of proxy form is required.）
What to bring	Certificate of notification of moving out (tenshutsushomeisho 転出証明書) from the place you moved from (if you have one) Notifier's personal ID (your passport, if moving from overseas) ※　Other things to bring if you have them Resident card or special permanent resident certificate Japanese pension booklet My Number card Basic residence register card
Useful notes	If you would like to enroll in National Health Insurance, please mention it at this time. You can apply even after 14 days have passed, but you must provide a reason for the delay, so please aim to do it on time.

Notification of moving out (when you move away from Oshu)

When to notify	14 days before moving out or within 14 days after moving out
Who can notify	The person who is moving or someone who shared their household in Oshu
What to bring	Notifier's personal ID ※　Other things to bring if you have them Personal seal registration card My Number card Basic residence register card National Health Insurance card Medical Insurance Card for the Latter-Stage Elderly Nursing care insurance card Medical expense subsidy card
Useful notes	Please have the new address and name of the head of household handy. Oshu will prepare your certificate of moving out, so please use that to submit your notice of moving in to your new city hall within 14 days of moving. It's possible to file a notice of moving out by post. See the downloadable application form at the bottom of this page for details.

Notification of change of address (when you move within Oshu to elsewhere in Oshu)

When to notify	Within 14 days of the move-in date (Procedures aren't possible before moving in.）
Who can notify	The person who is moving or a fellow household member
What to bring	Notifier's personal ID ※　Other things to bring if you have them Resident card or special permanent resident certificate My Number card Basic residence register card National Health Insurance card Medical Insurance Card for the Latter-Stage Elderly Nursing care insurance card Medical expense subsidy card

Procedures with your electricity, gas, phone, mailing address, NHK, and others may be required when you move. Please ensure that you contact these companies as needed.
