As of April 20th, 2023, the procedures for moving in/moving out/change of address at the window have changed. Two new services to notify a change in address – the Pre-Application Service and Desk Support – have been introduced. Please see the following file for details.

When to notify

Within 14 days of moving to Oshu

(Procedures aren't possible before moving in.)

Who can notify

The person who moved or someone in their household

(For a non-household member notifying on someone else's behalf, a notification of proxy form is required.)

What to bring

  • Certificate of notification of moving out (tenshutsushomeisho, 転出証明書) from the place you moved from (if you have one.) If you used your My Number card for moving-out procedures, you will need that instead of the moving-out certificate.

  • Notifier's personal ID (your passport, if moving from overseas)

(Caution) Other things to bring if you have them

  • Japanese pension booklet

  • My Number card

  • Basic residence register card

  • Resident card or special permanent resident certificate

Useful notes

  • If you would like to enroll in National Health Insurance, please mention it at this time.

  • You can apply even after 14 days have passed, but you must provide a reason for the delay, so please aim to do it on time.

  • My Number card
  • Basic residence register card
  • National Health Insurance card
  • Medical Insurance Card for the Latter-Stage Elderly
  • Nursing care insurance card
  • Medical expense subsidy card

Useful notes

  • Please have the new address and name of the head of household handy.

  • Oshu will prepare your certificate of moving out, so please use that to submit your notice of moving in to your new city hall within 14 days of moving.

  • It's possible to file a notice of moving out by post. See the downloadable application form at the bottom of this page for details.

Additional Note

Procedures with your electricity, gas, phone, mailing address, NHK, and others may be required when you move. Please ensure that you contact these companies as needed.
